You will be given an application number for each Form 470 that you create. Make sure you save this information. You will need it again!
You will also be able to create your own nickname to identify your form. It can be anything you want, but if you are filing multiple forms include something in the nickname that helps you distinguish between applications (e.g. type of service, funding year).
Make sure that you post for the correct category of service for the services you are requesting.
- USAC will deny a funding request if the category of service posted for the funding request on the FCC Form 471 does not match the category of service posted on the FCC Form 470
- Post for all applicable categories of service
- a service that can appear in more than one category should be posted in both
- a service that could be moved from one category of service to another during application review should be posted in both
Make sure you describe the desired products and services with sufficient specificity to enable interested parties to bid.
If you have a Request for Proposals (RFP) for the desired services, you will have to upload it into the EPC Portal during the process of completing your Form 470. It must be added before the Form 470 is submitted. Any additional document that provides information beyond what is provided in the Form 470 itself is considered by USAC to be an RFP (even though the document may not be what you consider to be an actual RFP).
Encompass all entities that will receive services - including non-instructional facilities.
The requested services listed are not binding, but services not listed on a Form 470 cannot be funded.
The form cannot be completed or certified by a service provider or feature a service provider as the contact person, and cannot be certified by a consultant.
