How to Tell a Form Has Been Filed

Each time a program form is filed, USAC issues a notification acknowledging receipt.

Save a copy of every form that you file and every notification letter from USAC, as well as other documentation for at least 10 years after the last date of service. Keep documentation for each funding year separate.
