For each FRN from each FCDL, you will need to provide:

• The service provider SPIN
• The service start date
• The status of compliance with the Children's Internet Protection Act (CIPA) for entities on that FRN

You will be given an application number for each Form 486 that you create.

You will also be able to create your own nickname to identify your form. It can be anything you want, but if you are filing multiple forms include something in the code that helps you distinguish between applications (e.g. type of service, service provider, funding year).

In order to truthfully certify your CIPA compliance on the Form 486, you should annually verify that all required procedures and associated documentation are in place.
